Site Description A 100-bed general emergency department and 8-bed behavioral health unit Patient Population Emergency patients co-managed with emergency department physicians. Majority of patients are on 5150 legal holds. Occasional delirium and dementia cases, substance intoxication and withdrawal. Site Description A 23-hour crisis stabilization unit (CSU) and a 50-bed psychiatric health facility (PHF). PHFs are a type of inpatient unit unique to California. Patient Population Uninsured and Medi-Cal (Medicaid)-insured Sacramento County residents who have severe mental illness. The majority of patients at MHTC suffer from psychotic and mood disorders (including schizophrenia, schizoaffective disorder and bipolar disorder). Site Description Adult community mental health clinic Patient Population Economically disadvantaged patients, including those uninsured and those insured through California's Medicaid program. The clinic provides for an ethnically diverse population including refugees from several countries and undocumented patients. The most common disorders are PTSD, mood, anxiety and psychotic disorders. Site Description Kaiser Permanente memory clinic, Sutter Health memory clinic, skilled nursing facilities Patient Population Seniors with neurodegenerative and comorbid mental health conditions. Kaiser Permanente is one of the largest managed care providers in Northern California (12.5 million patients). There are patients from all socioeconomic backgrounds (MediCal, Medi-Care, privately insured). Team PGY-2 resident, psychiatry or neurology attendings, social worker, psych RN, licensed psych tech Conferences and Learning Opportunities Residents can participate in our monthly case or impromptu conferences on the care coordination of complex patients, collaborating with a multidisciplinary team alongside their attending psychiatrist, case managers, social workers, nurses and psychiatric technicians. |
