Leukemia and Lymphoma Nurse Navigation for Patients with Cancers of the Blood
When facing the challenges associated with cancers of the blood, e.g., Acute Lymphoblastic Leukemia or Lymphoma, also referred to as a hematologic cancer, finding the right care and services you need can feel overwhelming. A nurse navigator works with you and your oncology team to help plan the best care for you in and out of the hospital.
UC Davis Comprehensive Cancer Center’s Leukemia and Lymphoma Nurse Navigator, working as part of Supportive Oncology and Survivorship, is here to help. An oncology nurse navigator is a professional registered nurse with oncology-specific clinical knowledge who offers individualized assistance to patients, families and caregivers to help address healthcare system barriers. A nurse navigator provides education, resources and referrals to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum (diagnosis, treatment, post-treatment, end-of-life care).
Our Leukemia and Lymphoma Nurse Navigator can help you with:
- Individualized and supportive care to patients and their families
- Coordinating elements of care, focusing on hospital to clinic transitions
- Assessing needs and barriers to care to help plan a way to reduce obstacles to uninterrupted oncology treatment
- Connecting patients and families to resources and help in your community
- Providing a bridge of communication between inpatient and outpatient medical teams
