24/7 Transfers and Emergency Consultations

Our registered nurses are available 24 hours a day, seven days a week to help coordinate urgent patient transfers between facilities.

  • We review transfer requests based on how urgent the case is and what resources are available
  • After hours, we also offer emergency phone consults for complex or unusual cases

Transfer and Admission Requests

All requests for acute facility transfers or direct admission will be considered for acceptance of care.

Non-urgent transfers are considered based on:

  • Available resources to meet the patient’s needs
  • Whether specialized (tertiary) care is needed
  • Teaching and research merits of the case
  • Other medical center commitments

Required Documentation
  • Medical Records

    Please send all required medical records with the patient, including:

    • Discharge summary
    • Imaging studies

    In some cases, we may ask you to fax records in advance for physician review. This helps us prepare the right care team and services.

  • Transfer Agreement

    To help manage resources and ensure continuity of care, we may request a transfer letter of agreement that includes:

    • An accepting physician
    • A receiving facility

    This enables the timely return of patients to their home community after completing specialized care.
