Options based on referral method you used:

• Electronic referral form: Submit directly to the specialty care clinic: Find clinic and contact information
• PhysicianConnect: Attach additional documents in the comments section of patient’s referral
• CERM (Epic): Attach additional documents to the comments section of patient’s referral
• 360X Referrals: Attach additional documents to a follow-up referral message for the patient’s referral in your EHR

For medical records, test results, or documents not tied to a referral, send to our Health Information Management (HIM) virtual file room at UC Davis Health.

• Patient full name
• Date of birth
• Address, phone number, or medical record number
