Referral checklist:

  • Completed UC Davis Health electronic referral intake form 
  • Recent/relevant clinical notes & test results (history, physical/well checks, imaging, labs)
  • Proof of insurance - Insurance we accept
  • Authorization information, including approved CPT codes, approved visits and dates

Note:

  • Incomplete information or need for clarification may delay referral processing.
  • Urgent self referrals are not accepted. Patients must obtain an urgent referral from their primary care provider.
  • This site uses end to end encryption, any data entered will be encrypted.

PhysicianConnect is a secure online portal that utilizes UC Davis Health’s Epic Systems web-based version of our electronic medical record system. This innovative application provides end users with real-time access to patient health information. PhysicianConnect allows UC Davis Health and its affiliates to maintain a strong working relationship through the effective use of communication and expedited access to the health information in relation to the patients we share.  

PhysicianConnect provides end users with the ability to view medical records of our shared patients and offers the ability to:

  • Create new charts 
  • Place referrals for services  
  • Check the status of referrals placed  
  • Place lab and imaging orders  
  • View upcoming appointments 
  • View current admissions  
  • Send in basket messages to staff

Using PhysicianConnect, our organization continues to pursue its mission to identify opportunities where we continue to improve the lives of the patients we serve by fostering innovative technologies, leveraging productive working relationships, and transforming health care by providing excellent patient care.  

For Epic users, Care Everywhere Referrals Management (CERM): allows you to track the entire referral process, from start to finish in addition to communicate with department referral coordinators.
To access:

  • Must be an Epic user
  • No need to attach additional supporting information
  • CERM provides the ability to track your referrals (i.e., confirm receipt, receive alerts when more information or additional documents are required, see when patients are scheduled or seen, receive automatic after-visit information, and communicate with UC Davis Health staff regarding the referral)
  • CERM functionality needs to be turned on by your organization

CHECK THE STATUS OF YOUR REFERRAL

You may contact the specialty care service directly, or you can check the status through your Physician Connect or CERM.

Call the requested specialty department:

Physician Connect: Login or register for our secure web-based system to view your referral status. Navigate to the comments section of your patient’s referral for updates or to send messages to the UCDH referral teams.

CERM: Login to your Epic EHR and navigate the comment section of your patient’s referral for updates or to send messages to the UCDH referral teams.

Providing additional records or authorization:

**Note: Prior to submitting a referral request, please ensure you have reviewed the Specialty Referral Guidelines to ensure all required records and authorized codes are included at the time of submission**

Webform: Send to the specialty department: Go here to find specialty clinics and contact information.

Physician Connect: Attach records or authorization to the referral comments section of patient’s referral.

CERM: If there are additional records or authorization that can’t be pulled via Care Everywhere, attach to the referral comments section of patient’s referral.

Non-referral related medical records, test results, etc.:

Forward to UC Davis Health Information Management Virtual File Room: virtualfileroom@health.ucdavis.edu.

All documents sent to HIM for review must contain patients full name, date of birth, and address, phone number, or medical record number.
