Industry Sponsored Clinical Trial Contract Packet Checklist

All clinical trial contract requests must be submitted through Cayuse.

Completed Internal Processing Form (IPF)

• The following must be uploaded in the IPF:
  
  • Sponsor Protocol
  • Clinical Trial Agreement draft (editable Word version)
  • Sponsor Budget
  • Internal Budget/Unified Budget Template (XLS)
  • Exception to Policy for Intellectual Property (Principal Investigator must sign) (PDF)
• Conflict of Interest in Research (COIR) Training (accessed through the UC Learning Center).
• COI Disclosures (700U and Form 800) (CAS)
  • Any questions regarding COI must be directed to the Research Compliance and Integrity office within the Office of Research directly.
• If applicable, complete the Principal Investigator's Statement of Exception to University Policy for Eligibility to Submit Proposals.
