How to Apply

To start the process of living kidney donation at UC Davis Health, please complete the Living Kidney Donor Questionnaire. After you submit it, the living donor team will review your information.

You must have a UC Davis MyChart account to complete this questionnaire. You can use your current account or create a new one. A simple How-To-Guide is available if you need help. If you have questions, you can contact the living donor team at (916) 734-2307.
